It's believed human error could have been one of the main factors in the fatal plane crash at Tokyo's Haneda Airport. Transcripts of air traffic communications show a passenger jet had been cleared to land when it collided with a coast guard plane that had not yet received permission to be on the runway. All passengers and crew on the burning airliner made it out alive, but five people on the smaller aircraft died.
